The Grampians Sector Development Team are funded to support HACC-PYP providers across the Grampians Region - Central Highlands and Wimmera, focusing on Service Collaboration.

In response to feedback from HACC-PYP Service Providers and DFFH, we will be hosting a one day in-person forum for HACC-PYP providers from the Central Highlands, Southwest/Wimmera and Barwon regions.

The forum will aim to build knowledge, create collaboration and networking opportunities and showcase best practice examples of HACC-PYP services.

The day will feature:

• Service co-ordination and pathways

• Data quality & reporting requirements

• Lived experience of service co-ordination: hear from Emma Gee

• Reflection and planning

• An opportunity to meet and network with other HACC-PYP providers from across the region
